Before D.C. Plane Crash, Army Helicopter Was Flying 100 Feet Too High and Twice Warned About Passenger Jet: Officials
Investigators revealed that the U.S. Army Black Hawk helicopter that collided with American Airlines Flight 5342 was flying about 100 feet higher than it should have been, and also stated the helicopter crew was warned about the incoming jet two times.

On Friday, the U.S. Army identified two of the soldiers in the helicopter: Staff Sgt. Ryan Austin O’Hara, 28, of Lilburn, Georgia, who is presumed deceased pending a positive identification, as well as Chief Warrant Officer 2 Andrew Loyd Eaves, 39, of Great Mills, Maryland.
